Replace direct adapter/persistence imports with context-based injection
(AdapterContext + useAdapters) so tests use in-memory implementations
instead of vi.mock. Migrate component tests from context mocking to
AllProviders with real hooks. Extract export/import logic from ActionBar
into useEncounterExportImport hook. Add bestiary-cache and
bestiary-index-adapter test suites. Raise adapter coverage thresholds
(68→80 lines, 56→62 branches).
77 test files, 891 tests, all passing.

Group current HP, temp HP, and max HP into a single bordered
pill container with a subtle slash separator. Removes the
scattered layout with separate elements and gaps. Temp HP +N
only renders when present (no invisible spacer).

3 integration tests render the full <App /> and exercise multi-component
flows: add/remove combatant, turn tracking across two combatants, and
HP adjustment with unconscious state. Add aria-label to the clickable HP
button so tests query accessible names instead of CSS classes.
